Unable to add or change a Contact in FSCM 9.1 MainMenu>Customers>Contact Info

Greetings,

Whenever we try to Add or Correct History any information within FSCM 9.1 Main Menu>Customers>Contact Information we receive the message below.  Has anyone already confronted this issue and resolved with success?
