PeopleSoft Billing - Excel Invoice upload. Cannot configure xls to include Note

Attempting to add a NOTE transaction to the Billing Invoice Excel Data Sheet.  The load is successful but no INTFC_BI_NOTE record is created.  Would like to see a xls an example of a  data sheet that is configured to input NOTE transactions .  Also are these notes header notes or line notes.
