Bell icon shows pending notifications, but worklist is blank

We are on 12.2.5.  A user reported that they had 5 notifications to approve transactions on their homepage worklist.  After approving 2 of the transactions, the worklist suddenly was blank.  However, the bell icon still showed 3 pending notifications.  He was able to approve them by accessing them via the bell icon.

(We have Beat By First Responder-style AME approvals, but no one else acted on these notifications)

Can someone suggest any causes/solutions?  If this is in the wrong community, please move to where it would be more appropriate.
