Error during ChallengeSMS Action OAAM (Access denied. Please try again.)

Hi,

I am trying to implement ChallengeSMS OTP using OAAM.

Whenever the OTP policy triggers and action is ChallengeSMS, user gets the following message: "Access denied. Please try again"

Details:

OAAM PS3

User is registered (Image and Challenge Questions).
