PS_INSTALLATION extra rows

We are running HCM 9.0 on PeopleTools 8.54.

Six months ago we started noticing extra rows in our PS_INSTALLATION table. 

We now get a new row about once per week.  It causes looping of code and results in performance issues.

The extra rows have default values and a blank company value.

We have a trigger in place now.  It shows the last two rows were inserted at different times by different user sessions while they were in module TR_EE_HR4_PNG with a last action of GVT_JOB0.

We interviewed the users and repeated their actions in a non production environment, but were not able to replicate the issue.
