IT Resource Configuration for Salesforce Connector

Hi,

I am installing the Salesforce Connector,  but as I am configuring the IT Resource  i have questions as to how it should be configured.

What values should go into the following fields:

authenticationServerUrl  (my thought is test.salesforce.com/services/oath/token)

host (again my thought is the speicifc host ala something.cs##.salesforce.com)

and what should the callback uri be on the connected app itself in salesforce?
