Workflow Error - WF_Dup_Role when trying to Rewind Workflow (Time-out)

I have a workflow that is in error Status
Error Name: WF_DUP_Role
Error Message: 395: Duplicate Role
This workflow tried to resend the notification 3 times and then appears to be 'Cancelled' when looking in Status Monitor. I attempted to 'rewind' the notification, however is giving me this error.
Can you advise on how to correct this and reactivate the workflow again?
