Price_Hist entry is missing

We are facing an issue while ranging new item loc in RMS.  Entries are successfully getting created in item_loc and item_loc_soh but fails to create in price_hist for tran_type=0.

Is this something that is a known issue? Also this issue is not happening with all the records.
