KK Ledgers Out of Sync

We have found that some of our KK Ledgers, including some Parent/Child ledgers, have become out of sync.  How to we determine the cause and correct this?

We are on 9.2, Image 20

Most of our Business Units have multiple KK Ledgers for reporting, some more than others. The ledgers roll-up differently, however all Expense transactions should be posting to all of them.  What causes them to become out of sync and how can it be corrected?
