How does notification button on portal header know user has pending approvals?

We are in the process of upgrading from pum10 to pum23 and wonder if there is some security or setup missing.

Here's the scenario:

User has pending approvals for requisitions

When user logs in the notification button does not tell user he has pending approvals. It is blank

The user can still get to their approvals by navigating to worklist, but not sure how the (flag) on the portal header works to tell user they have things to do
