Run-time error '429' with VoucherExcelToCI template

I have an AP staffer who has been using the VoucherExcelToCI template to load voucher data to staging tables for a number of months.  Today he is getting a Microsoft Visual Basic Run-time error '429' ActiveX component can't create object.  What do I need to look for to troubleshoot and resolve?  We are on 9.2 Image 18 PeopleTools 8.54.
