IEX: Staged Dunning Letter Template - Customization

Seeded IEX: Staged Dunning Letter Template doesn't meet our requirement. I need to change the table format slightly on the seeded template and add two new columns and remove couple of existing columns.

I duplicated the seeded template and changed the existing text and saved on to my desktop. I also duplicated the attached query to add the new columns required and saved. As per the instructions, I attached this query to my custom template.

Unable to make progress from here. How do I add the new columns to my custom template and reference them ? Is there any white paper or document that I can refer ? My search here for the last two days didn't show me a way.
