How do I use a URL attribute to launch an EBS form?

I created a custom workflow in EBS with a notification that contains a link to launch a form. The workflow is working and the notification is being sent. However, a message appears in the body of the notification: "3012: Illegal characters found"

I do not want this message to appear in the notification. I need to find out if it is possible to use a URL attribute instead of the form attribute to launch the form, and how can this be done.
