Oracle Forms/Report Applet with OID

Hello, have a quick question here.

I have installed the Oracle 11g Forms/report on a WLS server. I have hooked it up with an OID server that is also being used for EBS SSO.

So the next step I would like to do is to launch a Forms Applet and allows the user to be able to login via the Applet using his/her OID credentials.

Is there an existing Forms applet that comes with the base installation? If not, is there anywhere I can download an Form application and deploy it to the Forms server environment? So far I am not able to find anything wrt to how to do it.
