Tax Form Layouts for T4A & T4ANR in PS FSCM

We would like to generate the T4A & T4ANR tax forms directly from PS FSCM application, the data is present in a custom table and we would create a custom BI publisher to generate the tax forms, do you have the layout/template of these forms in PS FSCM?

We are on PS FSCM 9.2 (Image 14) and PT 8.54.16.
