Looking for input on configuring Chartfield Approval

Our organization upgraded to 9.2 in late 2016. Now that we've been on it a while, we want to start leveraging functionality we haven't used before, specifically Chartfield Approval. We want requests for new depts to route through the workflow approval. We've configured the base process in DEV using the configuration steps outlined in Peoplebooks. Unfortunately, we haven't been successful. Whenever we submit a new Chartfield Request, the Request Status is immediately set to "Newly Created". The new deptID is created without going through the approval process. There's no "View Approval Flow" link.  Has anyone else encountered the same issue?
