How do you add a custom field to BI Publisher (PDF version) when using XML as the data source?

We understand how to add a new field to BI Publisher when the source is a Query and the output is Word.  How do you add a new field to the XML source and modify a PDF version.  Do you need an add-in for BI Publisher in Adobe (as we already have for Word)?
