Error while creating BPA

Team - While creating a BPA in 12.2.6 environment, I am receiving the below error. Does anyone what could be the cause? Any inputs wourl be greatly helpful

oracle.apps.fnd.framework.OAException: java.sql.SQLException: ORA-01403: no data found

ORA-06512: at "APPS.PO_REQAPPROVAL_INIT1", line 261
