Dunning letter getting generated only for 1 customer

We are on R12 and using dunning letter since almost 5 years.

Today we tried to generate the dunning letters for delinquent customers and the system is only generating it for one customer. I ran the IEX: Send Dunning for Delinquent Customers for several times and I got out put for only 1 customer.

We had put a patch 22154709 to be able to send the customer number in the subject  of the dunning letter and tested it just for 1 customer and it worked fine. Now we are running it for everybody and getting this issue.

I am comparing the 2 environments where the patch is applied and not applied and found the following differences:
