Account Reconciliation - Need audit trail of who performed what action when

We are looking at the Account Reconciliation process in Demo.  We are trying to figure out how to see who is performing what actions on the Reconciliation record.  Seems like the PS_GLRN_AF_XREF record holds some important info. Not sure how to get to who did the Action.   I signed in as MGR1 and Denied the Recon.   Is there a record that holds that information?

We tried to look at the Account Reconciliation via the User Monitor for the Approvals - but it is not showing any info.

Any help would be greatly appreciated.
