Journal Workflow disappearing after submission

Hello,

We have been using workflow for a few years and suddenly during the month end process last month we were encountering a few users all describing a very similar situation.  Please see below for the specifics. 

Scenario: User submits JE for approval.  Reviewer receives email, entry is in workflow for their approval.  Upon reviewer opening workflow, JE appears with no person listed as submitter on the approval tab.  Reviewer approves and shows up as both submitter and approver on approval tab. JE does not post, and appears with blank submitter and approver in the system. 

To get through month end, we had the team delete those journals and try again.  The process would then work, although sometimes they would go through this scenario twice before finally allowing them to post the 3rd time.
