Adding Student Activity Guide Action Items

I am working on a custom Student Activity Guide based on the Program Registration Template.  In addition to the delivered action items, I have been asked to add action items to review Names, DOB, Gender, and Relationships.  The Names, DOB, and Gender could look like those in the Student Services Center.  The Relationships could look like those in the Person Profile in Contributor Relations.  Being new to activity guides, I would welcome any guidance on how to proceed to add these custom action items and services.
