Issue with Reviewer Worklist where WorkFlow is completed and user hits link

We have workflow where we have reviewer.  When the stage/step is set the reviewers get notified/worklist to view workflow. But they do nothing and the process continues when the Process gets approved/denied and finishes. The worklist entry is completed automaticly and add an entry in there work list to say it was approved/denied.

Problem the click the link and BAMM !!

     This error pops up

No approval process of id  is registered with the system.
