Query Manager Error - onReadyState

Our users are receiving an error when trying to run reports out of Query Manager.

The error reads:

"Function 'onReadyState' Error:  Unable to get property 'baseURI' of undefined or null reference

Error stack:

TypeError:  Unable to get property 'baseURI' of undefined or null reference at UpdateHistory....."

One of our users is consistently receiving this error.  Another user has mixed results (sometimes receiving the error, sometimes successfully running the query) depending on if she is using Internet Explorer or Chrome.

I have had the user clear their cache and they still received the same error in both Chrome and Internet Explorer.
