Send an alert or workflow notification if any parallel workflow notification is stopped due to any e

We have some requirement where we would like to send some kind of alert or workflow notification to one particular user or let's say initiator of workflow if in case that particular workflow stopped due to some system error or any issue with AME.

E.g. one Requisition was created by Requester and requester sends the Requisition for approval. There are total 4 levels of approvers for requisition and let's say at 2nd level there was some system error and in that case we would like initiator or any particular user of our choice to be notified either via some alert or workflow notification only. Is there any way to achieve this functionality? We are currently on Oracle R12.2.7.
