Old Items Appearing in Notifications

We have several users who have old/worked items (expense report approvals) showing up in their notifications.  If the user clicks on the link they get "This transaction has already been processed. (10504,592)"

I have gone to "Update Worklist Entries" and the items show up as "worked"

When querying the PSWORKLIST table the items' INSTSTATUS  status is '2'

Any where else I should be looking?
