How to build Document?

We need to create a document, using Document Technology,  with the following format.  There are several primitive elements and compound elements.  It will not us separate the primitive elements.  They have to be grouped together.

We are on PeopleTools 8.57.  How would you create a Document where the element names would be in this format?
