Is there a way to answer an activity guide questionnaire multiple times?

We are looking for a way to set up a questionnaire that would be answered daily.  It would take the place of a paper questionnaire about whether you have COVID symptoms or not.  Idea would be they could answer the questions at home, print out a doc showing they were cleared for work with date/time stamp, and then fill it out again the next day.

I feel confident on setting up the questionnaire and attaching it to an activity guide, but not sure how to repeat on a daily basis.  Would we have to create a questionnaire for each day?
