Tax rate error

Hello,

I am trying to create new tax rate in tax rate form but unfortunately getting the error ' APP-FND-01206: This record already exists. You entered a duplicate value or sequence of values that must be unique for every record.' due to this i am unable to create new tax rate.

Please help me to solve this error ASAP.
