Documentation on how to create custom Change Assistant templates?

We have a number of customizations, some of which don't neatly fit into an Application Designer project.  This started me on a path of researching whether and how to create custom templates for Change Assistant, so we can bundle things like Data Mover scripts or custom SQL together and apply them in an automated fashion.  I have not been able to find any comprehensive documentation to jump start my learning.  Does anyone know of such documentation?
