1099 Test Case : How to populate Box 16 , Box 17 and Box 18 on 1099 CopyB ?

I realize this is an old post, but I'm hoping someone can respond! As of calendar year 2018, we will be required to report in Boxes 16, 17, and 18 for the Sate of PA. We have created a manual process of entering State Tax credit vouchers and I believed this manual fix would work (as cumbersome as it is) for the actual reporting to the payee, but am now realizing these fields are completed at the vendor level. How can different amounts be entered for our multiple Withholding Control ID's? I see page 4 includes a screen shot of the fields for Tax Withheld and State Income, but the field to the right "Withholding Control Name" is not populated. Would populating this field solve our issue??

Since "duplicate" rows for IRS are not allowed, using the Withholding Name is not a solution. Please explain how to Populate Boxes 16-18 with amounts specific to each Control ID's creating 1099's.
