What information we need to create a new PeopleTools domain?

Hello, we have installed a new App server and another Web server, we need to create new App domain and New web domain to connect to the existing HCM database, can you please let me know what information we need from the excising env (menu below screenshot) in order to use/connect to the existing database? Thanks a lot!
