Need help with AWE escalations and notifications

I'm trying to create a calendar based escalation at 1 hr to the approver, at 2 hrs to the approver, and at 3 hours to an admin. I've read through PeopleBooks and the posts and article recommendations on here and have found the documentation to be lacking in specifics relating to how to define the Definition Notifications and Timeout Options and how the two work together. The NEM process is running successfully and the issue is that I cannot get the notifications to send to the correct people at the correct time. I've tried multiple setups and my latest is
