Peopletools 8.58 upgrade missing custom menus and content refs

We performed one successful peopletools upgrade from 8.56 to 8.58. The 2nd upgrade appeared to be successful. All of the upgrade jobs ran to success and the app server and PIA came up successfully. But when signing in, our custom menus and content references don't show up. And when clicking on Navigation, we get this error: cutom javascript error cannot set property 'tabindex' of undefined.

Has anyone seen anything like this?
