Fluid fields alignment issue

Hi All,

we are trying to develop a new custom ESS Self-service page on fluid. as we are doing first time facing issue with fields alignment on fluid page. could you please help me on stylesheet names to align to center the fields or remove the spaces . attached the page design here.

your help would be much appreciated.
