Question regarding Salesforce User form

in Identity Management (MOSC)

I am using 11g R2 Ps3 version of OIM. Client is using persistent UI as a front end of OIM. Received requirement to configure salesforce form and that attribute list can be accessed from Persistent UI. So anyone share idea about how to configure Form inherent functionality to access those attributes from other UI?. Thanks.
