Preliminary collections form - Operating Unit LOV is empty (CLE_F331_PRELCO.fmb)

We have set up Dunning Plans and would like to use the Preliminary collections form to review the invoices selected for dunning. However unable to use the Preliminary collections form to view these transactions. When the form is opened , OU is mandatory field and there is no value available in the OU LOV. Enclosed the profile values and error information for your review
