How to avoid RIB failres?

For last few days we are received few Poison family's rib failures. How to avoid poison family files failure in RIB. In fulfilord poison family we received some special character like '&', that character we removed then processed. But other family's files we just removed error lines and processed. Why the files are failed and how to avoid the rib failures?.
