OAM Custom Login page is always Blank

Hi,


I have a requirement to display a custom logging page for OAM when a user when a user redirected to OAM login page on accessing protected application URL. I have followed below attached document each and every step but its always showing blank page when redirecting to login page. Anyone please help me to resolve this issue.
