Questionnaire with approval

Hi,

We have a requirement where in the users would fill in a questionnaire pertaining to contract assessment. The filled in questionnaire would be reviewed by an approver, who based on the response would choose to approve or reject. Approval would forward the questionnaire to be reviewed by final approval. on that final approval, a contract creation event should be triggered. Notifications at each step is also required. To implement this, I should create an activity guide that will let approver review the filled in questionnaire, then approve or reject the step? If approved, move on to forward questionnaire to second
