Provisioning status of users from OIM to AD

Hi, I am facing an issue, deployed AD connector, while performing AD users provisioning from OIM to AD in specific OU, it is rejecting create process task and saying user already exist, But this is totally new user and not even existing in AD. But same time user is being created in AD in that OU, but status of account is in provisioning in OIM. Then I recon same user through target recon job, then new entry came in account with provisioned status.

Why user is going into provisioning 1st time and after recon it created. can anyone help me on this. Thanks.
