Custom Workflow is showing as "Not Found" at workflow history level.

Issue Description: we have developed custom workflow based on business requirement. the process is working as per expected. here the issue is at workflow history level, it is showing as "Not Found". this issue is not reproducible and intermittent only. not specific to user also. Am sharing both instance (Prod, latest clone of Prod) Screen shots. kindly advise how to troubleshoot and fix these kind of issues.
