Phase Out Item showed 'WIP completition' when made Active for few minutes.

We have situation where user wanted to return 1 unit in MFG for a Phase Out Item. When made it Active for the user to process the transaction for 1 Qty., there were 45 Qty. which had gone in to system for same Item and is showing 'WIP Completition'.
What could be the possible reason for this? How do we resolve this?
