Credit applications stuck in Submitted status, not creating credit case folders

We are experiencing the above issue and not sure how to proceed. Scoring model, recommendations, checklists all appear fine. Anyone have any thoughts on this as I don't recall us ever experiencing this. thanks

It is happening both from manually entered credit applications and from order management
