Why Free Gift Promotion not applied in SCA Kilimanjaro?

Hello! I have a question regarding promo code applications in SCA Kilimanjaro. Specifically, we are not able to get promo codes to be applied to orders where the promo code adds a discounted/free item to the cart. We have the following Free Gift Promotion:


In short, the promo will add a free item to orders over $250. We can add the promo to the cart anywhere; in the cart view or the in the mini-cart view during checkout. The promo code is visible, but the free item is never added to the cart. One thing that I've noticed is that during checkout, after selecting a shipping method but before applying the promo code I see:
