Hi User_AJB8Z, I have enabled all the features you have mentioned in my test account, the fields are still visible. Are you using the same form as with production account? Perhaps the form you are using is customized and the fields are not ticked to show.

Hi LLG, Can you please check the location affected (since reference key is 112 the location ID is probably 112 as well) if this is set to "inactive"? This error is commonly encountered if the location is tagged as "inactive". I hope this helps.
