Given that this seems to be a product issue, you may want to open an SR to have it be investigated.

Hi Connor, if something has gone wrong during the instance creation you may want to report the behavior to Oracle Support with detailed evidence. A behavior should be reproducible in order to investigate the cause. Now, in order to fix it, I suggest you recreate your instance in the correct domain.
