Trigger journeys task based on previous question response not working

Summary:

We have a requirement as below

HR - Provides response to a question (Yes/No)

Worker - A following task gets assigned to the worker based on response. (Yes)

However, when HR provides any response, the worker's task is getting marked as not applicable always. Anyone faced the similar issue and have any solution to it?

Any suggestions would be really appreciated.
