Multiple attachments are getting stored in DOR from Task with task type Report

Hi team,

We have created a task with task type Report and given the report path,selected the attachments dropdown and selected add the attachments dropdown to store the attachment uploaded by employee in a DOR Type.

When the employee clicks on See Document and then sign the document and upload it under attachment multiple files are automatically getting stored in DOR one is the report without signature and the second is the document uploaded by employee under attachment section.

Please help to resolve this issue.
