Journey - Add images to options

Hello all

I have seen in a demo from Oracle (Empower Yourself journey) that there is one task where the employee can select between two options. I assume this is done via a questionnaire (or configurable form) but I was wondering how you can add the images as per the below screenshot. Is there anyone who could help on this?
